Employment verification
Requests from a bank, a landlord or a future employer asking you to confirm somebody's employment. Dates of employment, designation held and whether the employment has ended can be confirmed without consent; anything beyond that cannot, and the screen keeps the two apart.

What the screen tells you
Dates of employment (joining and last working day) and Designation held on leaving and Whether the employment has ended may be confirmed with no consent at all. Pack §7.6 and §7.8: confirm dates and designation; anything more only with the ex-employee's consent.
This records only that somebody asked. Answering it is a separate, deliberate act, because deciding what may be disclosed is a decision and should not be a side effect of writing down that the phone rang.
Answering without the person's authority is a disclosure they did not agree to. Dates and designation may be confirmed without it. Anything more needs consent you can produce, and the field below is where you say where it came from.
Matching a request to a person is not answering it. It only says which record this is about, so the log can be read from the employee's side later.
